Pregnancy-Safe Skincare: The Two Questions That Replace the Whole List
How much gets into your bloodstream, and how much would that matter if it did? Separate those two and every confusing recommendation — including why retinoids and hydroquinone are avoided for completely different reasons — starts making sense.

Almost every pregnancy-skincare list is organized the same way: two columns, safe and unsafe, no reasons. That is useless the moment you pick up a product that is on neither list.
There is a better question, and it is the one the clinical literature actually asks: how much of this gets into your bloodstream, and how much would that matter if it did? Those are two separate axes, and almost every confusing recommendation makes sense once you separate them.
The absorption numbers, where they exist
Very few cosmetic ingredients have measured transdermal absorption figures. The ones that do are the most useful facts in this entire subject.
Hydroquinone has never been shown to harm a pregnancy, and no adverse effects have been reported. It is avoided because roughly a third of what you apply ends up inside you, and nobody has run the study that would tell us whether that matters. That is a precautionary call, and it is a reasonable one.

"Considered safe" in this field usually means nobody has reported a problem, not a trial demonstrated safety. The source that underpins most of this table says so directly: the data on adverse events in pregnancy is very limited, so it is reasonable to avoid products that have not been extensively studied. If you are the sort of person who would rather use four boring products for nine months, that is a completely defensible choice and you are not missing anything important.
The two things pregnancy actually does to skin
Most of the changes trace back to one cause: pregnancy raises androgen production, which drives oil, and hormonal shifts drive pigment. That produces two complaints, and they need different answers.

Sun exposure is the accelerant, and it is the only variable you fully control while pregnant. Daily broad-spectrum sunscreen, reapplied, plus a hat, will do more for pregnancy melasma than any treatment you are allowed to use — and it prevents the version that is far harder to fade afterward. The strongest treatments (hydroquinone, retinoids) come off the table for nine months, which makes prevention disproportionately valuable.
A routine that needs no decisions

- Treat fragrance-free as the default for anything that goes on a large area
- Expect acne and pigment to improve on their own after delivery
Don't
- Assume "natural," "clean," or "plant-based" implies pregnancy-safe — it implies nothing about absorption
- Book a chemical peel, laser or injectable "just this once"
- Panic about a retinoid you used before you knew you were pregnant — raise it with your provider instead
- Use a "safe during pregnancy" claim on packaging as your only check; the phrase is not regulated
- Start a new active in the third trimester when your skin is already reactive
Breastfeeding is a different question
The pregnancy list and the breastfeeding list are not the same, and people routinely carry the wrong one forward. The concerns shift from placental transfer to two things: what passes into milk, and what an infant's skin touches directly. Anything applied to the chest, and anything with meaningful systemic absorption, is worth re-checking with your provider rather than assuming the pregnancy answer still applies.

Everything else is a conversation with your provider, weighted by how much gets absorbed and how thin the data is.
